We are following CDC guidelines and we will be taking extra precautionary measures to protect the health of both you and our staff:

  COVID-19 symptom screenings of all patients

•   Temperature screenings

•   Enhanced cleaning and disinfection of all surfaces

•   Social distancing

As part of our new check in procedure to ensure everyone's safety, we will be asking you to:

•   Call us from the parking lot to announce your arrival
•   Wear a mask and bring your own pen in the event you have to sign any additional forms
•   Have your paperwork done prior to appointment by either faxing:

•   Please bring only essential family members (parents/ guardians). Limit caregivers that accompany those in need to 1 person per patient.
•   If you are sick, stay home and call us to reschedule. Do not come in if you have a fever, are not feeling well, or have had a recent exposure to someone with COVID-19.
